The REACH Center is open for walk-in services Monday thru Thursday during business hours and on Friday by appointment.

 Please stop in at the REACH Center and a Resource Advocate will work with you to determine what to do next.

Housing Assistance & Total Navigation Team (TNT)

If you are experiencing homelessness or housing insecurity, and are not currently working with a case manager or have lost contact with your case manager, stop into the REACH Center.

Coordinated Entry (CE) specialist - Our CE specialist can refer you to a community housing list. The community housing list will help you access housing openings in the community. This list combines housing opportunities from several organizations, in one place.

Total Navigation Team (TNT) - TNT meets on Tuesday and Thursday mornings at the REACH Center and is available to people who are not already working with a case manager but require housing support.

Tenant and Landlord Resource Office

If you are at risk of eviction, having a conflict with your landlord, or want to know more about your rights as a tenant, you can visit the REACH Center's Tenant and Landlord Resource Office.

Our Landlord Liaison can consult with you on how to work with your landlord to avoid eviction or manage other conflicts in a productive way.

If you have been issued an eviction notice, attorneys from Legal Action of Wisconsin are available onsite and through referral for a free consultation.

The Tenant and Landlord Resource Office also has educational resources for both landlords and tenants, and offers public computers for completing rental applications, housing searches, and meeting other online needs.

Hygiene & Non-Perishable Food Pantry

A hygiene and non-perishable food pantry is available on site. If you are in need of these items, simply come to the front desk and make your request.

Scenic Bluffs Community Health Services

Scenic Bluffs Community Health Centers provides behavioral health and substance use services at the REACH Center. In 2026, Scenic Bluffs will open a full clinic in the REACH Center, offering medical, dental, and behavioral health services — all in one convenient location to better serve the community.

Through this partnership, the REACH Center partners and Scenic Bluffs work together to provide wrap-around care, ensuring individuals and families have access to comprehensive support for their physical, behavioral, and social health needs.

The REACH Center staff can help you to:

  • Find a doctor or therapist that can work with you
  • Help make appointments
  • Help apply for Badgercare or access other insurance
  • Provide information about Long Term Care and help you to access the process to apply

Community Health Workers (CHWs) are also available to do all of the above as well as help you to understand health conditions and medications, advocate with you to health care professionals, help develop pro-active health plans, and provide assistance with accessing community resources to optimize your overall health and wellbeing.

Communication

Get help enrolling in the Lifeline program. The Lifeline program provides low-income Wisconsin residents affordable access to essential telecommunications by discounting the cost of phone, cell and internet services.
